Abstract Back ground: Acute-on-chronic liver failure (ACLF) is a syndrome characterized by acute decompensation (AD) of cirrhosis, organ/system failure(s) and high short-term mortality. Objectives: To assess cirrhotic patients hospitalized for an AD for the diagnosis and grading of ACLF and to predict short-term prognosis according to EASL-CLIF consortium to guide therapeutic decision. Patients and methods: 120 cirrhotic patients with AD were prospectively included. The patients were divided into ACLF and no ACLF patients according to EASL-CLIF Definition of ACLF. All patients followed up for 3 months from inclusion or mortality whichever was earlier. The Prognostic scores like CLIF-SOFA, CLIF-C OF, MELD, MELD-Na and child-pugh scores were evaluated for their ability to predict 28-d and 90-d mortality in all patients and patients with ACLF using AUROC.
