Abstract Rheumatoid arthritis (RA) is a disabling complex systemic autoimmune disease of unknown etiology where the close interface between soluble mediators and immune cells leads to intensification of chronic inflammatory and remodeling process that leads to proliferation of the synovial membrane of the small and major diarthrodial junctions and is associated with a worsening quality of life and reduced life expectancy. (1, 2) Some patients with early undifferentiated arthritis (UA), who are not fulfilling 1987 criteria, progress to RA. The definite mechanisms underlying the transition from UA to RA remain unknown. The early musculoskeletal (MSUS) examination together with synovial tissue analysis could have a role in disease diagnosis and prognosis. (3, 4) A study found that RA patients with low disease activity display low infiltration of T cells, B cells and neutrophils but pronounced macrophage infiltration. Thus, stratification of inflammatory arthritis in treatment naïve patients could be helpful to target therapy especially to patients with poor prognosis. |
