Abstract

The purpose of the current study was to compare the effect of transcranial direct current stimulation with task specific training in form of treadmill on proper gait pattern in children with diaplegic cerebral palsy. Subject: Forty diplegic CP children of both sexes ranged in age from six to ten years, their spasticity ranged from 1up to1+ grades in Modified Ashwarth Scale and Level I, II at GMFCS. They were divided randomly into two groups of equal number; group A & group B. Methods: All children of both groups were assessed for spatiotemporal gait parameters for 3 times; 1st assessment; pre treatment, 2nd assessment by the end of 2nd week; post-treatment 1 and 3rd assessment by the end of 12 successive weeks; post-treatment 2. Both groups received selected physical therapy gait training program for 5 times per week for two successive weeks and then 3 times per week for the remaining of treatment program for 10 weeks. group A received active transcranial direct current stimulation, group B received treadmill training, each intervention was applied for 5 times per week for two successive weeks (total of 10 sessions). Results: Pre and post-treatment findings of the current study revealed significant increase of both groups regarding spatiotemporal gait parameters, while no significant difference in group B when comparing post-treatment 1 to post-treatment 2 results. Comparison between groups showed significant difference in gait parameters post-treatments in favor to group A. Conclusion: Neuromodulation through transcranial direct current stimulation seems to be more effective than treadmill training with long lasting effect in improving gait pattern in children with diaplegic cerebral palsy
