Abstract

This study was conducted to assess diagnostic accuracy of a light induced fluorescence intraoral camera versus those of the visual-tactile assessment method according to modified ICDAS-II criteria clinically. Occlusal surfaces of 260 molar and premolar teeth from 52 adult patients who fulfilled the eligibility criteria were examined by two examiners using two diagnostic methods. The teeth were firstly assessed visually according to the criteria of the International Caries Detection and Assessment System (ICDAS-II) then by fluorescence camera. Inter observer agreement and intra observer agreement were measured for ICDAS- II and for the camera using Kappa test. Correlation between methods was calculated using Wilcoxon signed-rank test and effect size for comparison between the two modalities. The Sensitivity, specificity, predictive values, diagnostic accuracy, likelihood ratios, Area Under the ROC curve (AUC) and 95% confidence interval (95% CI) of the (AUC) for caries detection by Soprolife® were evalutaed. The results of the current study revealed a significant positive correlation between the ICDAS-II and the camera measurements. There was a statistically significant difference between scores of the two modalities (P-value <0.001, Effect size = 0.572). The sensitivity of Soprolife® was (94.2%), specificity (84.2%), positive predictive value (87.1%), negative predictive value (92.8%), positive likelihood ratio (6%), negative likelihood ratio (0.07%) and the diagnostic accuracy was (89.5%). Area under the ROC curve (AUC) was 0.909 with 95% Confidence Interval (0.863 {u2013} 0.955). For both ICDAs and Soprolife® regarding both observers; there was perfect intraobserver agreement (Kappa = 1.000) for both modalities and observers. Inter-observer agreement as regards ICDAs ; there was moderate inter-observer agreement (Kappa = 0.520). While for Soprolife®, there was substantial inter-observer agreement (Kappa =0.798).
