Abstract

Aim: The aim of this randomized clinical trial is to assess the effectiveness of advanced platelet rich fibrin (A-PRF) as compared to open flap debridement (OFD) in treatment of periodontal intraosseous defects in stage III periodontitis patients. Methodology: 22 sites in 22 patients, showing intrabony defects were selected and divided into intervention group where open flap debridement (OFD) was performed in conjunction with the application of advanced platelet rich fibrin (A-PRF) and control group: where open flap debridement (OFD) was performed alone. All sites were assessed for probing pocket depth (PPD), clinical attachment level (CAL), gingival recession depth (RD), full mouth plaque index (FMPI), full mouth bleeding index (FMBI), post-surgical patient satisfaction and radiographic bone fill. All parameters obtained at baseline, 3, 6 and 9 months and radiographic parameters were subjected to statistical analysis. Results: Both groups showed a statistically significant reduction in PPD and gain in CAL at 3, 6 and 9 months postoperatively compared to baseline values. The intervention group showed a mean PPD reduction of 3.36 ± 1.12 mm after 3 months, 3.64 ± 1.12 mm after 6 months, and 3.73 ± 1.19 mm after 9 months while the mean CAL gain was 3.18 ± 1.25 mm, 3.36 ± 1.12 mm, 3.55 ± 1.37 mm at 3, 6, and 9 months respectively. Concerning the control group, the mean PPD reduction was 2.00 ± 0.89 mm after 3 months, 2.09 ± 1.04 after 6 months and 2.18 ± 1.17 mm after 9 months while the mean CAL gain values reported were 2.27 ± 0.65 mm after 3 months, 2.36 ± 0.81 mm after 6 months and 2.45 ± 0.93 mm after 9 months. Both groups resulted in significant radiographic bone fill 1.62 ± 0.73 mm at 6 months and 2.09 ± 0.73 mm at 9 months for the intervention group and for control group, 1.07 ± 1.07 mm, 1.32 ± 1.21 mm at 6 months, 9 months respectively
