Abstract

Background: Vitiligo is an acquired, multifactorial disease of
the skin and mucous membrane characterized by depigmented macules.
It can be mainly differentiated into segmental and non-segmental
vitiligo. Diagnosis is mainly by clinical examination and wood’s light.
There are several treatment modalities among which are systemic and
local medications, phototherapy, lasers, surgical interventions and
camouflage. Dermoscopy is an important diagnostic and follow up tool
in dermatology clinics. In this study we assessed the value of
dermoscopy in evaluation of four different treatment modalities in
vitiligo patients.
Subjects and Methods: This study was carried out on 60
patients who were divided into four groups of 15 patients each
according to treatment modality used:
 Dermabrasion with Erbium Yag laser then application of 5Fluro-
Uracil cream.
 Topical Immunomodulator.
 Excimer light.
 NBUVB.
Four visits were documented for each patient (before treatment,
after one week, after one month and after 3 months).Photographic and
dermoscopic documentation were done each visit.
We decided to emphasize on five main findings (Erythema,
Hair color, repigmentation, peri-lesional hyperpigmentation and
telangiectasia).
Results: Before treatment, all patients of different groups (60
patients) combined showed a high statistically significant difference
between Photography and dermoscopy regarding hair color detection
and telangiectasia in favor of dermoscopy. Erythema showed
statistically significant difference between Photography and
dermoscopy after 1 st week with excimer light, and after 4 th and 12 th
week with dermabrasion+5fu.
Hair color showed statistically significant difference between
Photography and dermoscopy in the 1 st , 4 th , and 12 th weeks with
dermabrasion+5fu and excimer light. Telangiectasia showed
statistically significant difference between Photography and
dermoscopy in the 1 st , 4 th and 12 th weeks with dermabrasion+5fu, Elidel
cream and excimer light.There was also a high statistically significant
difference between dermabrasion+5fu in comparison with other groups
regarding erythema after 1 st week by both photography and
Dermoscopy.
There were also a couple of special findings along the way like
blue vitiligo, hair repigmentation and proof of false unresponsiveness
by naked eye.
