Abstract Summary and Conclusion This study was carried out to assess and compare the effect of implant position on occlusal load distribution in implant-assisted distal extension removable partial dentures. Computer guided occlusal adjustment and analysis was performed using the T- scan device. Twenty four patients exhibiting mandibular bilateral distal extension edentulous area (Kennedy class I) were precisely selected to participate in this study. The selected patients were divided into three equal groups:- group (I): For group I patients, a conventional removable partial denture was constructed restoring distal extension area. group (II): For group II patients, an implant-assisted distal extension removable partial denture was constructed with implant placed in first molar position. group (III): For group III patients, an implant-assisted distal extension removable partial denture was constructed with implant placed in second molar position. In an attempt to carry out a fair comparison and to assess and compare the effect of implant position on occlusal load distribution in implant-assisted distal extension removable partial denture, all patients received partial overdentures that were constructed following the same procedures, using the same materials and having the same design. At the time of denture insertion the T-Scan record was carried out to determines the contact time-sequencing, and the percentage of relative occlusal force between various occlusal contacts and to correct the occlusion. Follow up visits were scheduled, six, twelve months after denture insertion for occlusal analysis and adjustment using the T- scan device. Data obtained were collected, tabulated and statistically analysed. The results on the obtained results showed that: - Partial denture retained by two osseointegrated implants one on each side of the lower arch showed more stable occlusion compared by conventional partial dentures. - Implants placed at the second molar site provides more stable occlusal support and may be a more-effective location for implant-assisted distal extension removable partial denture compared with the first premolar site. - Changing implant position from first mandibular molar to second mandibular molar had minimal insignificant effect on the recorded time needed from the initial occlusal contact to achieve maximum intercuspation and the percentage of occlusal load distribution. - Computer-guided occlusal adjustments using T Scan provides multiple equal-intensity of occlusal contacts throughout the arches bilaterally. |
