Abstract

Background: Custodiol is an intracellular crystalloid cardioplegic solution used by some centres for myocardial protection in complex cardiac surgery and for organ preservation in transplant surgery. Histidine- tryptophan- ketoglutarate (HTK), Bretschneider’s, or Custodiol is attractive for cardiac surgeons because it is administered as a single dose and is claimed to offer myocardial protection for a period of up to three hours, allowing performance of complex procedures without interruption
Aim of the Work: The aim of the study is to compare the clinical outcome in patients with S-T elevation myocardial infarction (STEMI) who need early CABG, using two cardioplegic solutions (blood-crystalloid Vs custodiol).
Patients and Methods: A prospective comparative study conducted over a period of two years follow up for three month at departments of cardiothoracic surgery Ain shams university hospital and Minia University Hospital. for patients with STEMI are highly risky patients for CABG, with advancing of anesthesia and cardiac protection, the proper myocardial protection is suggestive for reducing mortality and morbidity for those patients.
Results: There is a significant difference between both groups regarding hyperlactemia and troponin level early post operative but no significant difference after 24hrs –and there are a significant difference between both group regarding arrythmia early intraoperative after declamping of the aorta. Also a significant difference between both groups regarding time of mechanical ventilation and ICU stay but no difference at total hospital stay.
Conclusion: Custodiol is a single shot cardioplegia, no need for re-administration which can disturb technical flow of the operation and no major significant difference between both groups regarding myocardial preservation during CABG
