Abstract

Summary Both RA and PsA are common inflammatory arthritis that can lead to irreversible joint damage and disability. Early diagnosis is a key to initiating early appropriate treatment, preventing joint damage and deformities and overall improved outcome. However, differentiating PsA sine psoriasis and sero-negative RA is a challenge due to the lack of a diagnostic biomarker and absence of specific psoriatic lesion. So, this study was conducted to evaluate the role of musculoskeletal ultrasonography and dermoscopy in the diagnosis of early psoriatic arthritis and early rheumatoid arthritis. Sixty Patients with early arthritis of hand joints (within 12 months of onset) diagnosed as either RA or PsA were enrolled in this study. 30 patients presented with early undifferentiated arthritis and after comprehensive rheumatological and dermatological evaluation besides MSUS and dermoscopy examination; 16 patients were diagnosed as early subclinical PsA (who showed US and dermoscopic findings of psoriasis) and 14 patients were diagnosed as seronegative RA. In addition to that, 15 patients with seropositive RA and 15 patients with PsA with evident psoriasis were enrolled as controls. Patients who were previously treated with systemic steroid therapy or conventional/biological DMARD during the previous three months were excluded from the study.
