Abstract

Background: : Most of physicians (pediatric and obstetrics )and nurses agree on the importance of breastfeeding and support breastfeeding promotion activities. However many physicians believe their training in breastfeeding management has been inadequate, and they lack confidence in their breastfeeding management abilities. In addition, some hospital infant feeding practices may impede breastfeeding promotion, as knowledge, attitude and practices of health workers can favorably or adversely affect breastfeeding practices among mothers, health professionals that women have contact with after giving birth. Furthermore, this first contact is a key opportunity to promote breast feeding . Aim of the study : To assess the knowledge and attitude of pediatric and obstetric physician and nurses related to breast feeding in Mansoura University hospital (MUH) and Mansoura University Children Hospitals (MUCH).Explore the implementation of Baby Friendly Hospital initiative in MUH &MUCH and assess pediatric and obstetric physicians and nurses’ knowledge and attitude towards this policy. Study venue : This study was conducted in Mansura University Children’s Hospital (NICU & General wards) and Mansura University Hospital (Obstetrics and gynecology department). Study design: descriptive study. Target population: The targeted study groups are Physicians)and nurses working at Mansura University Children’s Hospital, and Mansura University Hospital (Obstetric and gynecology department) Number of studied physicians and nurses:152. Methods: Interviewing the study subjects using WHO BFHI questionnaire to assess their knowledge and attitude towards BFHI. Recommendations: 1) Hospitals need to establish national standards and guidelines for the protection, promotion and support for breastfeeding in all facilities providing maternity and newborn services, based on the updated Ten Steps for Successful Breastfeeding. 2) Health-care worker should have the competencies to implement the BFHI, in all health care facilities that have maternity and neonatal care services. 3) We found that identification cards about ten steps of breastfeeding at NICU and Obstetric OPC and absence of this policy at general words. So we need to support this policy and to Confirm the importance of ten steps of breastfeeding by identification cards and workshops with health care workers to gain the best result for supporting breastfeeding. 4) Regular internal monitoring is a crucial to establish both quality improvement and ongoing quality assurance. 5)External assessment is a valuable tool for validating the quality of maternity and newborn services. External assessments should be sufficiently streamlined into existing mechanisms that can be implemented sustainably and follow up assessment of knowledge and attitude towards breastfeeding regularly. 6)Regular training programs for health care worker providing update knowledge and supporting of positive attitude towards breastfeeding and its importance and to try to solve problem facing them in implanting step of BFH. 7) Social media should be included as it has great effect on breast feeding initiation and promotion, we should create online course to improve breast feeding knowledge and treat false concept about it.
