Abstract

This comparative prospective study was conducted with the aim to evaluate the safety and efficacy of combined phacoemulsification and SubCyclo (subliminal transscleral cyclophotocoagulation) in the management of glaucoma patients with concomitant cataract.
The study included 42 eyes of 42 patients diagnosed with mild to moderate glaucoma. The eyes were divided into two groups. group 1 underwent combined phacoemulsification with SubCyclo (subliminal transscleral cyclophotocoagulation) laser procedure in the same setting, while group 2 underwent phacoemulsification only.
Operative and postoperative data were recorded and analyzed to detect any statistical difference.
The study looked at the intraocular pressure reduction and the number of intraocular pressure-reducing medications in both groups.
It also looked at the postoperative complications encountered in group 1 and the safety profile of combining phacoemulsification with subliminal transscleral cyclophotocoagulation
The IOP and number of glaucoma medications were measured at baseline as well as at 1 day, 1 week, 1 month, 3 months, and 6 months post-operatively.
The mean percent reduction of the IOP in the combined phacoemulsification + SubCyclo group at the final follow up visit of the study was 16.09%, with a mean intraocular pressure of 14.00 mmHg ± 4.71 at the final postoperative visit compared to 17.90 mmHg ± 5.14, this was found to be statistically significant.
The mean percent reduction of the IOP in the phacoemulsification stand-alone group at the final follow-up visit of the study was 16.07%, with a mean intraocular pressure of 15.71 mmHg ± 3.31 at the final postoperative visit compared to 19.14 mmHg ± 3.62, this was also found statistically significant.
The mean number of IOP-lowering medications was found to be statistically significantly lower in the combined group at the final follow up (1.43 ± 1.36) compared to the mean number of IOP-lowering medications pre-operatively (2.95 ± 0.97)
On the other hand, the mean number of intraocular pressure-lowering medications was found to be statistically significantly higher in the control group at the final follow up (1.43 ± 0.87) compared to the mean number of intraocular pressure-lowering medications pre-operatively (0.90 ± 0.88)
