Abstract

Aim of work:
Aim of this work is to study right and left atrial deformation changes in patient with non valvular AF using two dimensional speckle tracking echocardiography.
Patients:
This study had been carried on 25 patients with documented non-valvular AF, and 25 healthy subjects without cardiovascular disease, hypertension, or diabetes mellitus had been enrolled as controls.
We included Patient with long standing persistent AF, AF duration > 1 year. We excluded CHD, valvular AF, patient with cardiogenic shock or hemodynamic instability.
Each included patient was subjected to full history taking, thorough clinical examination, 12 lead Electrocardiogram, conventional 2D echo and 2D STE. Conventional Echocardiography measuring: end diastolic dimension and volume (LVIDd, and EDV), end systolic dimension and volume (LVIDs and ESV), Fractional shortening (FS), EF, septal and posterior wall thickness (LVSWT and LVPWT), E/e’, RA and LA diameters and biplane volume.
Offline analysis of raw ultrasound data was performed at Menoufia University - Department of Cardiology using GE Echopac and the following views were taken for analysis; apical 4 chamber view, apical 2 chamber view, and apical long axis view. LA, RA and LV strain (ε) and strain rate (SR) for each wall were quantified and comparison between Patients and control groups was performed.
Results:
Our results described that:
- LA and RA diameters and volumes were detected to be statistically significantly higher in the AF group in comparison to the control group.
- LVESD, Mitral E, E/é septal and E/é lateral were detected to be statistically significantly higher in the AF group in comparison to the control group, in contrast the FS% and EF were detected to be statistically significantly higher in control healthy group.
- LA and RA strain and strain rate were significantly impaired in AF patient in comparison to normal healthy controls.
- LV GLS and SR were significantly impaired in AF patient in comparison to normal healthy controls.
- RA strains for the septum (basal, mid and apical), mean septum, the lateral wall (basal, mid and apical) and mean lateral wall, in comparison to the LA strains for the same walls and there was no statistically significant difference between both strains in the AF Patients.
