Abstract

Rheumatic heart diseases are the result of an autoimmune response to pharyngitis. The long-term damage to cardiac valves caused by ARF, which can result from a single severe episode or from multiple recurrent episodes of the illness, is known as RHD and is a notable cause of morbidity and mortality in resource-poor settings around the world (Carapetis, et al, 2016).
The aim of the current study was to assess the quality of life for primary school aged children suffering from rheumatic heart diseases through assessing the physical functions, psychological functions and social functions that affected by the disease.
Research questions:
1. What are the effect of rheumatic heart diseases on physical domains for primary school child with this diseases?
2. What are the effect of rheumatic heart diseases on psychological domains for primary school child with this diseases?
3. What are the effect of rheumatic heart diseases on social functions for primary school child with this diseases?
4. What are the factors that affect the QOL for school age children suffering from rheumatic heart diseases?
Research design: a descriptive analytical research design was used in order to meet the aim of the study.
Setting: the study was conducted at the cardiac outpatient clinic in health insurance at Nasr city, the rational for selecting the setting having the highest admission rate among the insurance place for selected aged children from hospital records.
Sample: -A purposive sample includes 50% from 300 of total number of school age children suffering from rheumatic heart diseases who are attending the cardiac outpatient clinic for receiving the prophylactic medication in 2011, 2012 and 2013 from hospital records. The actual study sample was 150 school aged children, inclusive criteria from 6 to 12 years diagnostic with rheumatic heart diseases without any other physical and/or mental comorbidity.
Tools for data collection:
It was written in simple Arabic language and consisted of four parts:
Part (1): Socio-demographic characteristics of primary school children.
Part (II): Disease characteristic of primary school children.
Part (III): Mothers’ knowledge regarding to RHD.
Part (V): Quality of life for school age children suffering from RHD. Rating scale adopted form (Verny, 2006). The quality of life rating scale was used to determine the level of QOL of children suffering from rheumatic heart diseases by measuring essential life spheres physical, psychological and social functions as well as general well being.
Result: The main result of the study revealed that:
• The study showed that less than half of children their age in 10 - <12 years 420.0%. More than half of them females 55.3%, more than half of them second child 53.0% and three quarters of children were < 5 number of family member 74.7%
• Less than three quarters of children had two rooms 74.0% and more than three quarters of children lived in good housing ventilation 78.0%.
• The majority of children were daily school attendance was 98.0%.
• Less than three quarters of their fathers and their mothers were university education 73.3% and 72.7% respectively
• Less than three quarters of their fathers were employee 72.0%, and less than two third of their mothers were worked 59.3%.
• The past history of children showed that, less than half of them had the disease for 1- >3 years 43.3% and two fifth of them had previous admission to hospital 40.7%.
• Signs and symptoms of children, most of them had tonsillitis and sore throat 79.3% and less than three quarter had fever, and dyspnea 72.0% &71.3% respectively
• Present history of studied children, the most of them had low concentration 82.7%, and more than three quarter of children had Chest pain 78.0% and less than three quarter of them had Palpitation with sleep or rest 69.3%.
• The mother’s knowledge about rheumatic heart disease the most of them knew that, the diseases appearing from the age of 5 years to 15 years, preventive treatment with antibiotic s will continue until the child reaches the age of 18 years and malnutrition in children 86.7, 86.0 &84.0% respectively.
• Level of prevention of RHD, nearly two third of their mothers knew that improve the living and social status in primary prevention of RHD 64.7% and more than half of the mothers knew that secondary prevention through use of antibiotic treatment (penicillin) each month 55.3%.
• More than half of their mothers had satisfactory knowledge about RHD 55.3%.
• More than half of the studied children had moderate physical condition 56.0%.
• More than two fifth of the studied children had low psychological condition 41.3%.
• Less than half of the studied children had moderate social functions condition 44.0%.
• There was a statistically significant difference between QOL and age, houseing ventilation, number of family and mothers jobs
