Abstract

This study conducted on 100 patients who were admitted in Coronary Care Unit of Sohag University Hospital with Acute coronary syndrome.
We excluded any patients on antiplatelet and anticoagulant therapy , with bleeding tendency , on corticosteroid therapy or with history of surgery in the past 30 days .
All patients were subjected to: full history taking, clinical examination , Laboratory investigations (WBC with differential count , Neutrophil/lymphocyte ratio (NLR) was calculated , platelet count , mean platelet volume (MPV) , hemoglobin level, Red Cell Distribution Width ( RDW), Lipid profile , Renal function ,Liver function ,Cardiac enzymes.
The study demonstrated that age of our studied groups ranging from 33-90 years old and 68 % were males and 32 were females. As regard risk factors of ACS 37% were hypertensive, 33% were diabetic, 66% were smoker and 45% have family history of IHD. 66% of our studied groups were STEMI , 16% were NSTEMI and 18%were unstable angina . 39% of studied population developed cardiovascular complication. Neutrophil/lymphocyte ratio, mean platelet volume, hemoglobin level , Red Cell Distribution Width have value in predicting outcome in patients with ACS.
The primary findings of this study suggest that a simple, relatively inexpensive, and universally available test can be used to predict cardiovascular complication in patients with ACS. High neutrophil with low lymphocytes is significantly correlated with the risk of increased short term complication and major adverse cardiovascular events. NLR emerging as an independent predictor of cardiac mortality in patients with ACS.
MPV, which is an index of platelet size that correlates with platelet activation and is a reliable index of platelet activation, may be a potentially useful marker in cardiovascular risk stratification . The study show that higher MPV indicates an increased risk ischemic complications.
In this study , Anemia adversely affect prognosis of ACS patients as it exacerbates myocardial ischemia due to decrease blood oxygen saturation needed to meet myocardial requirements.
This study has demonstrated that RDW is associated with risk for MACEs in patients diagnosed with ACS. Relatively affordable, convenient, and readily available, RDW can be used as a risk stratification tool in our resource-limited setting.
This study should draw attention that patients with high risk to develop cardiovascular complication need intensification of medical treatment, good control of risk factors and early angiographic intervention.
