Abstract

Comorbid anxiety disorders are common among patients with schizophrenia .This phenomenon is neglected by a lot of clinicians, so the outcome and the quality of life of the patients affected negatively. There aren’t sufficient studies for this phenomenon. We aimed to assess this phenomenon in such an important area in Egypt like Ismailia. We put a primary objective to assess the prevalence and the severity of the comorbid anxiety disorders among our sample of patients with schizophrenia. We also aimed to study the relation between anxiety disorders and schizophrenia and if there is gender difference regarding this comorbidity.
To achieve our objectives, we carried out our descriptive cross section study at Suez Canal university hospital in Ismailia governorate. Our sample was 63 patients who had been diagnosed by a consultant psychiatrist using DSM 5 criteria as schizophrenic patients and we included only those who attended the outpatient clinics for follow up as well as who were cooperative for the interview.
We excluded those who had a history of: neurological disorders, major physical conditions and psychoactive substance abusers. And those who were in acute psychosis or mental retarded.
We used socio-demographic questionnaire and extracted demographic characters for detecting the relations between them and the comorbidity. Severity of psychotic symptoms had been measured and quantified with the Positive and Negative Syndrome Scale (PANSS) in all patients. Semi structured interview using DSM-5 had been administrated to diagnose anxiety disorders including generalized anxiety
disorder, social phobia, panic disorder, specific phobias and separation anxiety disorder then Hamilton Hamilton Anxiety Rating Scale for anxiety (HAM-A) had been administrated to detect the severity of anxiety. We extracted clinical data about family history, number of hospitalizations, compliance with treatment, age at onset of schizophrenia and its duration.
Our Results Summarized that:
Prevalence of comorbid anxiety was 57.1% among patients with schizophrenia distributed as follow: panic disorders = 22.2%, generalized anxiety disorder (GAD)= 14.29% ,social phobia= 9.5% , more than one anxiety disorders=11.11%.
Regarding socio-demographics:
● Sex: Our sample included 33 males (52.4%) and 30 females (47.6%).
● Age: Mean age was 36 y and the majority was from the age of 18 to 45y.
● Level of education: (52.4%) were of moderate level of education.
● Occupation: Unemployed patients represented (65.1%).
● Marital status: There was (79.4%) not currently married and (20.6%) currently married.
There was no significant association between socio demographics and existence of comorbid anxiety except marital status which showed statistical significance.
Regarding variable clinical factors:
● Family history: It was negative in (68.8%) and positive in (30.2%
) of the sample.
