Abstract

The study aimed at assessing the current use and practice of ECT in Dakahlia through the following objectives: the schedule of ECT administration, diagnoses for which it is used and indications for ECT at these diagnoses, the methods in which ECT is applied: (modified or unmodified), (square wave pulse or sine-wave current), device type, electrode placement (bilateral or unilateral), anesthesia, maintenance ECT and outcome of treatment with ECT. The study subjects included: All psychiatrists on duty at Mansoura university hospital and Demeera hospital after excluding members-in-training (residents and fellows). There were 30 psychiatrists during the duration of the study. The current study used the following methods: All participants went through semi structured questionnaire designed for research. This questionnaire is APA task force questionnaire (American Psychiatric Association, 2001) which is modified to be suitable with our study. The Current study revealed that: Total number of patients seen by participants in 6 months duration was 9520 patients, from them 439 patients were recommended to be treated with ECT this means that 4.6% of psychiatric patients in MUH and Demeera hospital received ECT. As regard attitudes toward ECT there were 96.7% of participants have some degree of favorable attitude. Major depression disorder is the most common indication for ECT in Dakahlia, as reported by participants, followed by manic excitement then schizophrenia. Technique of administration of ECT in MUH and MHH is nearly the same as regarding the use of short-acting anesthetic drug which applied by an anesthesiologist before administering ECT and muscle- relaxant drug is administered before ECT. Type of machine used is Mecta and the type of current used is bidirectional square wave pulse (ultra brief in MUH, pulse width 0.3 ms and brief, pulse width 1.5 ms in Demeera Mental Hospital). The most common electrode placement technique is BL, followed by RUL and combined BL and RUL. The most common treatment schedule was twice a week in Dakahlia. Participating psychiatrists reported that schizophrenic patients require average 8sessions for remission; major depressions require 7; and minor depressions require 5. Maintenance ECT is used commonly once a month.
