Abstract

Myomectomy is the method of choice for removing myoma in patient who want to preserve fertility, however massive bleeding is one of it’s Complications. Myomectomy is also performed frequently in infertility with the presence of fibroids.
Misoprostol is a synthetic analogue of natural prostaglandin that was approved by the US Food and Drug Administration. The myometrial contractions caused by misoprostol applications in Obstetrics and gynecology.
The aim of this study was to compare the amount of intraoperative blood loss in cases performing abdominal myomectomy operations preceded by vaginal administration of misoprostol 400ug one hour before operation versus cases underwent myomectomy without receiving this drug.
This study was performed postmenstrually on 50 women were diagnosed having single uterine leiomyoma and were admitted in Gynecology department to perform myomectomy operation. The age of the patients ranged from 30 to 45 years, all were fit for surgery, where the main complaints are abnormal uterine bleeding, dysmenorrhea and pelvic pain.
The patients encluded in the study were randomly assigned into two equal groups (25 cases in each group). group I was administered one dose of 400 micrograms misoprostol one hour before operation (Misoprostol group). group 11 underwent abdominal myomectomy without preoperative misoprostol (control group).

Regarding our results, there was no significant difference between 2 demographic data, clinical presentations and size of leiomyoma. estimated blood loss intraoperative in group I (misoprostol group) compared to the blood loss (ml) in group II (control group) was with statistically significant difference between two groups.It was found that intraoperative blood loss in (ml) was lower in misoprostol group than in control group. Also the results of haemoglobin (g/dl) postoperative show statistically significant difference between the two groups as the mean of haemoglobin in misoprostol group (11.36 0.59) which was higher than haemoglobin in control group (10.04+ 0.35). Also the values of hematocrit postoperative show significant differences between two groups. The mean of hematocrit in misoprostol group was higher (36.89+2.66) than in control group (32.06+1.35).
As regard the operative time (in minutes), it was found that the time in misoprostol group (56.8 ±3.12) was less than the time in control group (78.6 ± 10.6) with a statistically significant difference between two groups.
Regarding the recorded postoperative complications there was no statistically significant differences between two studied groups.
In this study it was investigated that a single dose of vaginal misoprostol 400Mg one hour before myomectomy would reduce intraoperative blood loss and, operative time and need for blood transfusion.
