Abstract

world. It is a safe procedure, but occasionally it can cause severe complications
can lead to blood transfusions, surgery, hospitalization, or rarely, death.
Inadequate bowel preparation is a common cause of poor quality colonoscopy.
For patients requiring colonoscopy while admitted to hospital, achieving adequate
cleansing of the colon is often difficult. Regardless of indication, the success of
colonoscopy is closely related to adequate colon preparation (David, 2020).
The purpose of the current study was to examine the effect of nursing
intervention on bowel clearance, controlling complications and satisfaction among
patients undergoing colonoscopy.
Subjects and method:
To achieve the aim of the current study a qausi experimental research design
was utilized. The current study was conducted at Gastrointestinal Endoscopy Unit,
at Menoufia University Hospital. Data were collected over a period of 5 months
extended from the beginning of October 2019 to the end of February 2020.
 Consecutive sample of 60 adult patients of both sexes who was admitted to
the endoscopy unit and scheduled for colonoscopy divided randomly and
alternatively into two equal groups 30 patients in each group (study group
and control group).
 Study group (I) : who will receive a detailed education about proper
technique of colon preparation for colonoscopy by using designed video
and colored material illustrates all these instructions and activities along
with routine medical care.

 Control group (II): who will expose to routine hospital care only.
Instruments of the study:
Four tools were carried out for data collection of the current study
Instrument I: Structured interviewing questionnaire:-
This tool was developed by the researcher to assess sociodemographic and
knowledge of the patients. It was written in English and was comprised of two
parts:
Part one: Demographic data: It was designed to collect specific
information of study sample such as: age, gender, marital status, occupation, level
of education, residence.
Part two: Medical data: It include questions about medical history and
reason for colonoscopy.
Instrument II: Boston Bowel Preparation Scale (BBPS): The Boston
bowel preparation scale is the most extensively validated scale to assess the
quality of bowel preparation for colonoscopy. It was developed by the section of
gastroenterology at Boston Medical Center (BMC). Adopted by Lai, et al. (2011)
to distinguish various degrees of bowel cleanliness..
Instrument III: selected Colonoscopy’s Complications Assessment
Checklist: This tool was developed by the researcher, based on previous review
of literature. It used to assess post colonoscopy complications such as abdominal
pain, abdominal distension, chest pain, tachycardia, hypotension, nausea and
fever.
Instrument IV: Self-Administered Questionnaire 9(Modified Group
Health Association of America 9 (mGHAA-9 Questionnaire): It was adopted
by (Johanson, et al., 2000). The questionnaire used was based on the modified

group Health Association of America9 (mGHAA-9) questionnaire but with the
question on technical skill of endoscopist being replaced by a question on patient
comfort level during colonoscopy and the addition of a question on comfort level
during bowel preparation and will use by researcher to assess patient satisfaction.
It consist of ten questions that will reflect degree of satisfaction.
The main findings of current study were:-
There were highly statistically significant and clinically relevant
improvement in bowel clearance and satisfaction after implementing educational
nursing intervention with less incidence of colonoscopy problems and
complications to patients in the study group than control group.
Conclusions:
Based on the findings of current study; it can be concluded that
implementing educational nursing intervention using a designed video and simple
booklet had a positive effect to patient for enhancing quality of bowel clearance,
improvements in patients‘ satisfaction and reducing colonoscopy‘ problems and
complications for patients undergoing colonoscopy.
Recommendations:
Educational nursing intervention using written and visual information as
booklet and designed video should be carried out to facilitate educating patients
about proper preparation of the bowel before colonoscopy, Sufficient information
about signs and symptoms of the potential complications and the importance of
seeking rapid medical advice after colonoscopy.
