Abstract

This study was done to evaluate the root canal cleaning after conservative versus the traditional endodontic access opening cavity preparation. One hundred first maxillary premolars were used in this study. Teeth were cleaned, autoclaved and then 4 specimens were randomly chosen to serve as sterilization control. The remaining specimens were randomly divided into four equal experimental groups of 25 specimens each, according to the design of access opening carried out and then infected with Enterococcus faecalis suspension for four weeks.
The 4 groups were: group I: conservative endodontic access with chemo-mechanical preparation and passive ultrasonic irrigation. group II: conventional endodontic access with chemo-mechanical preparation and passive ultrasonic irrigation group III: conservative endodontic access without chemo-mechanical preparation. group IV: conventional endodontic access without chemo-mechanical preparation. Following this, teeth were fixed in formalin solution and submitted for the Brown and Brenn stain for the detection of gram-positive bacteria.
The infected surface area in each slide was calculated by TIF score in order to compare the percent of the bacterial reduction in relation to the negative control.
The results were collected, tabulated and statistically analyzed. In the coronal third, group I had the least score of TIF followed by group II. While at the middle third of the root canal system, group II scored the least TIF score followed by group I without statistical significance

in the two situations. While a statistical significance was found when comparing those two groups to their negative control counter-groups (p= <0.001),
While in the apical third, group II was believed to be statistically superior to group I (p=<0.001) regarding the decrease in TIF score when compared to the baseline TIF score of the two negative control groups.
