Abstract Background: Postoperative sore throat and hoarseness of voice are common complications following endotracheal intubation for general anaesthesia. These adverse events may have a negative effect on patients’ satisfaction and activities after surgery. The objective of the current study is to evaluate the efficacy of inhalation of budesonide Vs intravenous Dexamethasone in reducing the incidence of postoperative sore throat and hoarseness of voice in patients undergoing elective surgery under general anaesthesia. Methods: This is a comparative randomized clinical study performed in Faculty of Medicine, Fayoum University on 120 patients ASA I and II patients scheduled for an elective surgery with tracheal intubation under general anaesthesia. Pre- anesthesia evaluation was performed and then selected patients was divided into three groups each group include 40 patients, group (A) inhalational budesonide group, group (B) intravenous dexamethasone group and group (C) Control group. At the end of surgery, Patients were transferred to postoperative anesthesia care unit (PACU) with standard monitoring applied. Outcome assessment for the incidence of sore throat and hoarseness was carried out at 1, 6, 12 and 24 hours post- extubation Results: There was no statistically significant difference between the three groups regarding their sex, age, weight, height and BMI. Sore throat was significantly lower in the dexamethasone group than the budesonide group and the control group after one and six hours and it was continuing to be low but with no statistical significance after twelve and twenty four hours. Hoarseness of voice was significantly lower in the dexamethasone group than the budesonide group and the control group after six hours only and it was continuing to be low but with no statistical significance after twelve and twenty four hours. Conclusions: This study revealed that the incidence and severity of post- operative sore throat and hoarseness of voice were lower in patients received IV dexamethasone than those treated with budesonide by inhalation prior to induction of anaesthesia with comparable results in relation to the risk factors as BMI, number of intubation attempt , time to achieve intubation and duration of the procedure.
