Abstract

Penetrating keratoplasty is a corneal transplant procedure in which full thickness host corneal tissue is replaced with donor corneal tissue.PK is considered one of the commonest solid organ transplantation with less rate of immunological graft rejection. This can be attributed to corneal immune privilege. Although PK is an effective treatment for corneal diseases with poor vision, the survival of the graft depends mostly on many factors. These factors can be divided into three categories. First are donor factors including donor age, donor sex, cause of death, death to preservation time, endothelial cell density, and storage duration. Second are recipient factors including recipient age, recipient sex, indication for the procedure, associated corneal vascularization and pre-exciting glaucoma. Finally, the operative and postoperative factors including small or large graft, eccentric grafts, suturing technique, graft recipient disparity, suture loosening, longer operating times, also a previous anterior segment surgery, active inflammation or infection at the time of surgery, and occurrence of postoperative complications and rejection episodes. The aim of this study was to assess indications, visual and refractive outcome, topographic changes, endothelial cell analysis, AS-OCT findings, graft survival results and graft clarity in patients undergoing primary conventional PK in Tanta University Ophthalmology Hospital.Moreover, to address the different complications and risk factors affecting graft survival following the operation.
