Abstract

Parathyroid hormone is thought to be an important player on the muscle mass beside its effect on the bone, kidney and the intestine .It also regulates calcium, phosphorous and vitamin D level in patients with chronic kidney disease .Many studies showed that high parathyroid hormone levels is associated with decreased muscle mass in different mechanisms. The aim of the current work was to study the correlation between parathyroid hormone and the muscle mass in hemodialysis patients. This study was carried out on 81 patients treated by regular hemodialysis over a year. Medical history and complete clinical examination were done to all of them. Also the following laboratory investigations were done: serum parathyroid, calcium, phosphorous, alkaline phosphatase, complete blood picture, serum iron, serum ferritin, total iron binding capacity, TSAT, pre and post hemodialysis urea and creatinine, arterial blood gas, serum albumin, cholesterol, serum total protein, serum Na, K, C reactive protein, hepatitis C virus. Creatinine generation rate, normalized protein catabolic rate and kt/v were calculated. Moreover, anthropometric measurements were done like body weight, height, BMI, waist circumference, mid arm muscle circumference, mid arm circumference, triceps skin fold, fat free mass, fractional fat free mass, total fat percentage. Protein energy wasting (PEW), Dialysis malnutrition score (DMS) and malnutrition inflammation score (MIS) were also taken.
