Abstract

Background: A 2006 cochrane review examined the use of caulopyllum, cimicifuga, and other natural remedies, but found a lack of sufficient evidence to show effectiveness of natural remedies as a method of induction. The review looked at two studies comparing natural remedies and placebo for cervical ripening or labor induction, and found that while the mean length of labor was shorter in the control group in both studies,, small sample sizes and insufficient detail in the research made it impossible to draw any meaningful clinical conclusions. More rigorous, adequately-powered studies are needed to evaluate clinically meaningful outcomes, adverse effects such as shypertonic uterus, and indicators of maternal and neonatal morbidity.
Objective: To determine, the effectiveness of Black Cohosh for third trimester cervical ripening, induction of labour, safety and dose adjustment.
Patient and methods:
The current prospective study was conducted on 100 primiparous women planned for cervical ripening and trial of labor at Ain Shams University Maternity Hospital during the period between December 2018 and December 2019. All women had singleton term, viable preganncies, vertex presentation. They were scheduled to undergo cervical ripening and induction of labor at ≥ 39 weeks < 42 weeks gestation. They were subdivided into 2 equal groups each of which had 50 pregnant women and were randomized to treatment with either [black-Cohosh 1 femrose capsule 80 mg / day orally for 48 hrs, ”group A” and black-Cohosh 2 femrose capsules 160 mg/day orally for 48 hrs” group B”].
Results: This current study found that a dose of 160mg black-cohosh 2 Femrose capsules /day orally for 48hrs is a safe and an effective regimen in achievement of cervical ripening, reduction in the induction-to-delivery interval, in the rate of need and dose of oxytocin for augmentation of labor and in increasing the number of vaginal deliveries.
Conclution: This current study found that a dose of 160mg black-cohosh (2femrose capsules) /day orally for 48hrs is a safe and an effective regimen in achievement of cervical ripening, reduction in the induction-to-delivery interval, in the rate of need and dose of oxytocin for augmentation of labor and in increasing the number of vaginal deliveries.
