Abstract

This study was performed to determine the effect and outcome of calculated nutritional regimen on critically ill COPD patients in comparison to patients received conventional nutrition and for assessment of nutritional needs that suitable for their stressful condition.
To achieve this purpose, , this study included 40 COPD patients who were intubated and mechanically ventilated in respiratory ICU of Chest Department in Assiut University Hospitals between Mai 2016 and December 2017. Patients were divided into 2 groups: Group1, who received conventional nutrition regimen and Group2, who received adjusted and calculated regimen. Patients with conventional nutrition had mean age of 66.35 ± 10.17 years while mean age of those patients with calculated nutrition was 69.80 ± 5.32 years with insignificant difference in between both group (P= 0.18). Majority of patients (80% of those with conventional nutrition and 75% of those with calculated nutrition) were males.Patients were subjected to careful history taking,general and chest examination,Estimation of height, Ideal body weight and BMI ,complete blood count, electrolytes, Albumin level,kidney function tests, and Spirometry.
We found asignficant relation between nutritional management, duration of mechanical ventilation and outcome in critically ill COPD patients (duration of mechanical ventilation was significantly prolonged in those patients on conventional nutrition in comparison to those with calculated nutrition) and (75% of those with calculated nutrition had simple weaning while no weaning occurred in majority (50%) of patients with conventional therapy).
