Abstract

This cross-sectional prospective, case-control hospital based study was conducted in Internal Medicine Department, Minia University Hospital from June 2016 to June 2017.
This study will be carried out to assess the presence of ascitic fluid RANTES and its value in diagnosis of spontaneous bacterial peritonitis SBP among (HCV) cirrhotic patients
This study included 80 subjects who were recruited from inpatient and out - patient clinic and divided in to two groups:
group 1: It included 50 patients with HCV-related liver cirrhosis complicated with SBP. Diagnosis of liver cirrhosis is based on abdominal ultrasound & laboratory data .Diagnosis of SBP is based on PMNLs count (>250 cell/mm3), patients will receive treatment for SBP in the form of third-generation cephalosporins ( cefotaxime) as the first-line treatment (2g/8hours) then follow up of the patients by clinical examination and measuring RANTES level in ascitic fluid will be done to evaluate effect of treatment on its level.
group 2 : It included 30 patients with HCV-related liver cirrhosis not complicated with SBP.
Diagnosis of HCV based on positive anti-HCV antibodies by enzyme- linked immunosorbent assay (ELISA) and/ or detection of serum HCV-RNA by polymerase chain reaction.
Subjects of group 1 and group 2 were be age and gender matched.
The study proved that level of RANTES was significantly higher in cirrhotic patients complicated with SBP compared with cirrhotic patients without SBP and its level decrease after treatment of infection.
The current study showed that bilirubin level , the concentration of the total protein in ascitic fluid , decreased pro coagulant factors , Child-Pugh stage C are risk factors for Spontaneous bacterial peritonitis.
In current research , the level of SAAG was significantly higher in SBP patients.This study concluded that total leucocytic count and the ascitic fluid PMN count were significantly higher in spontaneous bacterial peritonitis . The degree of ascites was significantly higher in SPB group.
In this study, there were positive association between RANTES and ascitic fluid PMN count before and after treatment .The level of RANTES before treatment and ascitic fluid total protein concentration.
This study showed that RANTES level was significantly higher in both groups with body mass index.
