Abstract

Gastric cancer is one of the most common malignant tumours and the second leading cause of cancer death worldwide. Laparoscopic gastrectomy has been used as a superior alternative to open gastrectomy to treat early gastric cancer. Although still in debate, the application of laparoscopic surgery in advanced gastric cancer has drawn increasing attention over the years.
This study was conducted on 40 patients diagnosed having distal gastric carcinoma at Menoufia University Hospitals between August 2016 and September 2019.
The results showed that, no significant difference between both groups in age or gender (p=0.9 &0.2 respectively).
Operative time was significantly prolonged in LP group than open group (180.2 ± 72.1 min versus 150.7 ± 28.4 min, p=0.04). Also, Blood loss in open group significantly higher than LP group (420.8 ± 87.6 ml versus 283.4 ± 45.9 ml, p<0.001).
Also, we showed the required parenteral analgesia in open group (4.9 ± 2.1 d) was significantly higher than LP group (3.1 ± 0.5 d) (p=0.04). Time to start oral feeding and Hospital stay in open group was (5.8 ± 1.9 d & 8.8 ± 3.2 d respectively) while in LP group was (4.1 ± 1.1 d & 6.9 ± 1.6 d) with p value 0.03 &0.02 respectively.
On the other hand, there was no significant difference between both groups in Wound infection, Wound dehiscence, Leak, Bleeding and Mortality (P value=0.3).
And finally, the total number of LN was 23.9 ± 4.2 and 24.1 ± 3.9 in Open & Laparoscopic group respectively, while the Positive
