Abstract

VM is considered one of the most common vestibular disorders in the general population and probably represents the second most common cause of vertigo after benign paroxysmal positional vertigo.
The aim of this study was to study vestibular function in VM patients and to compare between the efficiency of cinnarizine and topiramate as prophylactic therapy for vestibular migraine. In this randomized clinical trial, 60 patients with age range (18-60 years) were recruited from Audio vestibular Outpatient Clinic at Menoufia university hospitals, diagnosed as Vestibular Migraine patients according to ICHD criteria for vestibular migraine.
The included patients were submitted to full history taking, otoscopic examination, audiological evaluation, vestibular evaluation in the form of videonystagmography. Subjects included in this study were divided into 2 groups: Cinnarizine group, consisted of 30 cases and Topiramate group, consisted of 30 cases.
Cinnarizine (dose of 25 mg / 8 hours) and topiramate (dose of 25 mg / 12 hours) were given to the patients for 3 months. The efficacy of treatment was serially assessed at baseline, 2 weeks, 1 month and 3 months after treatment as regards the frequency, duration and severity of headache and vertigo. In addition, frequency of side effects related to cinnarizine or topiramate treatment were evaluated.
The current study demonstrated that cinnarizine and topiramate treatments resulted in significant reduction in monthly vertigo/migraine frequency, duration and intensity from the baseline through 12 weeks of treatment, 1 month, 3 months. The reduction in mean monthly vertigo /migraine frequency and intensity showed no significant differences for cinnarizine versus topiramate treatments from the baseline through 12 weeks of treatment, 1 month, 3 months. With regard to the safety measures, cinnarizine was better tolerated and frequency of side effects was less in cinnarizine group.
