Abstract

Growing number of geriatric people around the world in addition to the increase in the life expectancy of individuals led to an increase in the number of elderly people undergoing surgical procedures including colostomy that is a surgery performed to connect a portion of the colon to the front abdominal wall where the waste is passed through an artificial opening. Patients with colostomy face both physical and psychological complications which had negative effect on psychological status and reduce the patient’s self esteem. Therefore, educating and training the geriatric patients how to take care of stoma increases their independence which leads to improve their psychological status and self esteem. Aim of the study: This study aimed to determine the effect of stoma care training program on self esteem and psychological status of colostomy geriatric patients. Materials and Method: A Quasi- Experimental design was used in this study. The study was carried out at inpatient surgical wards and in colorectal and anal unit in Mansoura university hospital . The calculated sample size was 45 patients attending the previously mentioned settings. Data was collected using 6 tools; demographic and clinical data structured interview schedule, geriatric patients colostomy knowledge structured interview schedule, Stoma care observational checklist, Rosenberg Self – Esteem Scale (RSES), Hospital Anxiety and Depression Scale (HADS) , The Mini Mental State Examination (MMSE) Scale (1975). Results: Results revealed that colorectal cancer was the main cause of colostomy. On the other hands, permanent colostomy was the most common types of colostomy. Total knowledge scores and total practice scores of geriatric patients increased significantly immediately after and one month after implementation of the training program , In relation to patients’ psychological status, decreasing in anxiety and depression level were observed immediately after and one month after implementation of the training program .Also, statistical significant strong negative correlation between both total knowledge and practices scores and either total anxiety scores & total depression scores and total self esteem mean score of the patients immediately after and one month after program implementation. Recommendation: Based on the results of the present study there is an increasing need to develop and implement psychological rehabilitation programs for colostomy geriatric patients to improve their psychological status.
