Abstract

Nasal turbinates are important anatomical and physiological landmark structures of the lateral nasal wall. Concha Bullosa (pneumatized turbinate) is a phenomenon which is considered one of the most common anatomical variants of the sinonasal region which usually occurring at the middle turbinates more than other nasal turbinates . The exact reason of the pneumatization of the middle turbinate in some people but not in others, is still unclear, but this phenomenon has been shown to be of clinical importance as its incidence was about 38% of people who had been prepared for radiological nasal examination, according to some studies.
The vast majority of cases is asymptomatic, but the excessive enlargement of the middle turbinate pneumatization can lead to persistent nasal symptoms that not responding to various medications. It may also impair the access of intraoperative nasoendoscopy. The definitive treatment of these cases is mainly surgical by various techniques as ; lateral laminectomy (partial turbinectomy), conchoplasty (turbinoplasty) and other techniques.
The aim of this study was to compare between lateral laminectomy and conchoplasty , as regard their outcome results .
This prospective study was carried out on 40 patients aged over 18 years who have clinical and radiological evidence of symptomatic middle turbinate concha bullosa when they presented to the outpatient clinic of Otorhinolaryngology department of Menoufia university hospital from October2016 till April 2018. Patients were divided equally into group A for lateral laminectomy and group B for conchoplasty.
In this study , the preoperative assessment of the main complaing symptoms (contact rhinogenic headache and nasal obstruction ) was done by taking full clinical history and examination including using CT nose and paranasal sinuses ,nasoendoscopy and evaluation tests (topical xylocaine, VAS and SNOT22).The surgical techniques were done according to the standard description in the literature.
