Abstract Summary Iron is an essential mineral necessary for oxygen transport through hemoglobin in red blood cells and for oxidation by cells through the cytochrome. Iron is needed by body to synthesize its oxygen transport protein especially hemoglobin and myoglobin and to form heme enzymes and other iron-containing enzymes. Iron deficiency anemia is considered the most common micronutrient deficiency among children worldwide of prevalence is dependent on the economic status; however, no socioeconomic group is spared. Simple nutritional iron deficiency anemia can be easily treated by nutritional programs and oral iron therapeutic agents with good clinical and hematological responses after the proper period of time; but unfortionately refractoriness to oral iron therapy may occur and this is usually due to several underlining causes ranging from simple avoidable causes to more advanced and need to investigate ones. Unresponsiveness to oral iron therapy may be due to poor patient compliance, inadequate iron dose, duration or preparation, incorrect diagnosis, or due to presence of coexistant disease and underlying deficiency.
