Abstract

Introduction: Various treatment modalities used for anterior disc displacement including cognitive behavioral therapy, physical therapy, pharmacological therapy, and intraoral appliances. This study was conducted to evaluate the impact of intra-articular ozone gas injection in contrast to conventional splint therapy as treatment modalities for internal derangement of the TMJ. Method: A total of 20 patients with clinical signs and symptoms of anterior disk displacement without reduction were selected from the outpatient clinic of faculty of dentistry, Mansoura University. All patients were divided randomly and equally into two groups. In 1st group ten patients received ozone injection twice weekly for three weeks injected in the Superior Joint Space (SJS). While, in 2nd group ten patients received anterior reposition splint in upper arch for 24 hours/ three months. All patients were assessed clinically regarding pain, inter-incisal distance, right and left transition after 2,4,8,and 12 weeks. M.R.I were used to evaluate disk –condyle relationship either preoperatively and postoperatively after 6 months. Result: A high statistical significant differences were recorded between both groups regarding to inter-incisal distance at different time intervals of follow up (P=0.001, 0.0001, 0.0001, 0.002 and 0.001 receptively). While, no statistical significant differences regarding pain were recorded for same time intervals of assessment (P=0.3, 1, 0.4, and 0.2 receptively) except at the 8thweek (P= P=0.005).However, a high significant difference was recorded between both groups after 6 months of follow up at M.R.I. (P=0.006). Conclusion: Although, ozone therapy can provide a short term palliative effect in treatment of anterior disk displacement in comparison with anterior repositioning splint however, it has a limited mechanical recapture role to establish normal condylar disk relationship.
