Abstract

Hypertensive disorders are the most common medical complication of pregnancy. The overall prevalence of hypertension during pregnancy ranges between 5% - 10% globally , and increases in developing countries such as Asia, Africa, Latin America and the Caribbean was reported to range from 10% - 15%.
The present study was designed to assess the (prevalence of hypertensive disorders of pregnancy, maternal morbidity and mortality in a population based hospital in Upper Egypt. Also other parameters e.g. age proportion and delivery methods and other related outcomes in the major referral hospital at El-minia governorate, Egypt that serve about five and half million populations and located in the Upper Egypt to enable us to provide a preliminary data to our policy makers and community managers to what extent the magnitude of such a problem in our community and if there is a difference regarding the incidence and spectrum of complications than other parts of the word.
The aim of this work was to evaluate the incidence of HDP in women attending to El Minia university materiality and children hospital, and also to evaluate maternal outcome (mortality and morbidity), provide statistical study to clarify risk factors that reflect the disease’s complexity, Evaluation of treatment and make some recommendations to try to control the disease.
The results of the present analysis included the following:
In our pilot study the total number of deliveries during January 2016 till December 2016 was 11247 patients, 1338 cases suffered from hypertensive disorders during pregnancy. The reported incidence of HDP was 11.9%.
The effect of maternal age is one of the most important demographic factor that affects the incidence of HDP as 37.5% of cases were below the age of 25 years, (39.5%) of cases were ranging from 25-35 years and 23% of the cases were more than 35 years.
Antenatal care was insufficient and inefficient as only 53.5% of cases received antenatal care more than > 4 visits and only 40.8% of patients were diagnosed and treated.
In the present study, the incidence of mild preeclampsia was diagnosed in 339 cases (25.3%) of hypertensive women, and severe preeclampsia 307 cases (22.9%).
In the present study 56 women were chronic hypertension, Gestational hypertension cases were 261 (19.5%) of HDP cases.
In the present study we had 12.2% eclampsia (166) cases, HELLP syndrome (63) cases (4.7%), 32 cases had stroke (2.4%).
The frequency of caesarian section in the study was 72.4%.
In the present study, the proportional rate of maternal mortality by HDP was 45.5% 10 deaths out of 22 deaths and case fatality rate was 0.75%.
In the present study death was more in Primigravida (70%) of all deaths and also ICU admission was (56%) of all cases admitted in ICU.
In the present study that hypertensive disorders were the main reason for admission to the intensive care unit by (48.6%).
In the current study, hospital received more pregnant women with hypertension coming from rural area (66.8%) than urban area (33.2%).
