Abstract

Cancer is a fundamental aberration in cellular behavior. Effective and reliable venous access is one of the cornerstones of modern medical therapy in oncology. The management of a patient with cancer demands stable venous access that is used for a wide range of underlying diseases, including chemotherapy, blood product and antibiotic administration, and fluid resuscitation (Jayasekara, MacInnis, Room & English, 2016).
Port-A-Cath., is an implanted venous access device,it is a thin, soft, hollow plastic. Put under the skin. It has an opening just under the skin. The port is a disc about 2.5–4cm (1–1.5in) in diameter. It can be used to give chemotherapy or medications into the vein, or to draw blood. Nurse’s responsibilities for implanted port. This is a critically important task. Access/ de-access/re-access I.P. Perform proper flushing technique. Use the facility’s procedure for drawing blood (Rostom, 2015).
Aim of the study
The aim of this study is to assess nurses’ performance regarding care of patients with implanted port undergoing chemotherapy through:
• Assessing nurses’ knowledge regarding the care of patients with implanted port undergoing chemotherapy.
• Assessing nurses’ practice regarding care of patients with implanted port undergoing chemotherapy.
• Assessing nurses’ attitude regarding care of patients with implanted port undergoing chemotherapy.
Research questions:
This study was conducted for answering the following questions:
• What is the level of nurses’ knowledge regarding the care of patients with implanted port undergoing chemotherapy?
• What is the level of nurses’ practice regarding care of patients with implanted port undergoing chemotherapy?
• What is the nurse’ attitude regarding care of patients with implanted port undergoing chemotherapy?
Research design
Descriptive exploratory design was utilized in this study.
Research setting
The study was conducted in an oncology department in the EL - Demerdash Hospital affiliated to Ain Shams University.
Subjects
A covenant sample of all nurses (50 nurses) working in the above mentioned setting was involved in this study.
Tools for data collection:-
1. Self-administered questionnaire: it consists of three tools:
- Demograbphic characteristic data of nurses under study
- Nurses’ Knowledge assessment form regarding care of patients with implanted port undergoing chemotherapy.
- Factors affecting nursing performance regarding care of patients with implanted port undergoing chemotherapy.
2. Nurses practice observational checklists: to assess nurses’ practice regarding caring for patients with implanted port.
3. Likert scale: to assess nurses’ attitude regarding patients with an implanted port undergoing chemotherapy.
Results
The present study revealed that:
The total level of nurses’ knowledge about all aspects of implanted port including; knowledge regarding cancer disease, chemotherapy, administer chemotherapy, implanted port, care of implanted port before, during and after its implantation, complications of implanted port and infection control of implanted port the result revealed that 72% of nurses had an unsatisfactory level of knowledge regarding caring for patients with implanted port undergoing chemotherapy.
The total level of nurses’ practice on all aspects of implanted port as; flushing, recapping, dressing, preparation chemotherapy, administration chemotherapy and infection control of implanted port the result revealed that 78% of nurses had an unsatisfactory level of practice regarding caring for patients with implanted port undergoing chemotherapy.
The total level of nurses’ attitude about all aspects of implanted port the study revealed that half of nurses of the current study had appositive attitude, and about one third of them didn’t know the correct attitude toward care of patients with an implanted port undergoing chemotherapy. While the other nurses about less than one quarter had a negative attitude regarding care of patient with an implanted port undergoing chemotherapy.
There was a highly statistically significant relation between age and knowledge also age and practice of nurses under the study and there was a statistically significant relation between nurses knowledge & their experience, also statistically significant relation between nurses practice & their qualifications and experience in chemotherapy unit.
There was a highly statistically significant relation between overall nurses` knowledge and practice regarding care of patients with an implanted port undergoing chemotherapy.
Conclusion
Based on the findings of the current study, this study concluded: More than three quarter of the studied nurses had disagreed opinion about factors affecting their performance. More than two third of the studied nurses had unsatisfactory knowledge with statistical significance difference regarding the care of patients with implanted port undergoing chemotherapy. More than three quarter of the sample had unsatisfactory practice with high statistical significance. While half of the studied nurses had a positive attitude regarding care of patients with implanted port undergoing chemotherapy. Results revealed that there was a statistically significant relation between total nurses’ knowledge and practice as regards their demographic characteristics. Also, there was a statistically significant relation between total nurses’ knowledge and practice.
