Abstract

Burn scars are supposed to be a dermal fibrotic disorder where red, raised, pruritic lesions distort the skin .Burn scar are cosmetically disfiguring and force the scarred person to deal with an alteration in the body appearance, resulting in delayed return to work and reintegration into the society. Socially, burn patients may experience staring and pointing by others and the impact of looking different on interactions with others has been well documented. There is no standard option for treatment of scars, scarring still remains a therapeutic challenge in plastic surgery. There are a variety of scars and treatment options that can be used to achieve significant cosmetic improvement, but it must be noted that none of the currently available treatments can achieve complete resolution of the scar.
In this study we evaluate the effect of applying platelet rich plasma on post burn scars clinically and histopathologically.
Twenty patients with post burn scars were enrolled in this study; the twenty Patients completed the whole treatment and follow up protocol. In each patient the affected area was injected by platelet rich plasma intralesionally with applying microneedling device (dermapen) into areas of post burn scar. Four sessions, 2 weeks apart were performed. Needle size 2 mm,36 needle tip was used. Stamping technique was used. The session was done until enough bleeding was seen.
The study revealed very highly significant reduction after PRP and microneedling treatment therapy (P value <0,001) according to modified Vancouver Scar Scale .
Histopathological evaluation using H&E stain grading score of dermal papillary collagen revealed a significant improvement of collagen fiber following PRP and microneedling treatment.
Finally, the current study used platelet rich plasma to treat post burn scars, it was able to achieve more abundant collagen deposition. Such positive histopathological and tissue changes were reflected clinically on the appearance of the scars.
