Abstract

Background: Proximal femoral fractures, more generally termed ’hip fractures’, are among the most common injuries. Femoral intertrochanteric fractures, accounting for about 50% of hip fractures, are one of the common fractures in elderly patients. Intramedullary fixation and extramedullary fixation are surgical techniques for the treatment of intertrochanteric fractures. Systematic review of literature done to evaluate which technique is better.
Methods: 11 studies (from 2000-2017) were included and reviewed the results of 1,625 patients (836 patients in the intramedullary group and 789 patients in the extramedullary group) in terms of: Blood loss, Blood transfusion, operative time, hospital stay, functional scores, re-operation rates, mortality rate and complications (intraoperative risks, later fracture, implant failure, non-union, superficial wound infection and screw cutout.
Results: Intramedullary fixation method showed decreased amount of blood loss, blood transfusion requirements, shorter operative time and hospital stay duration. Also the intramedullary group showed higher functional scores than the extramedullary group. Regarding complications: the intramedullary group showed a significant lower difference regarding the implant failure rate. However, there was no statistical significant difference in terms of intraoperative, later fracture, non-union, superficial wound infection and screw cut out.
Conclusion: it was concluded that the intramedullary method is preferred on the extramedullary method for internal fixation of the unstable intertrochanteric femoral fractures.
