Abstract Chronic lymphocytic leukemia is one of the most common lymphoid malignancies, accounting for approximately 11% of all hematologic neoplasm’s. The CLL cell usually express membrane phenotype as (CD5+ve,CD23+ve,CD10-ve), mantle cell lymphoma usually expressing (CD5+ve,CD22+ve,CD23-ve), hairy cell leukemia usually expressing(CD5-ve,CD23-ve,CD103+ve) and follicular lymphoma usually express ( CD5-ve,CD10+ve,CD23-ve) , so the clinical and phenotypic overlapping occurs among B- cell lymphoma subgroups . CD200: is a type-1 membrane glycoprotein belonging to the immunoglobulin super family, expressed by dendritic cell, neurons and subsets of T-cell , B-cells precursors and mature B- cells , this glycoprotein not expressed by normal plasma cells and NK cells . the aim of this study was to study the role of CD200 in differential diagnosis of mature B-cell neoplasm’s. The study included 60 patients(49 male and 11 female) divided into four groups Their age ranged from ( in CLL ,38 - 65 years) , (Atypical CLL: 40-70 years),(MCL, 45-70years), (other NHL, 45-78 years). The results showed the following : - highly significant elevation in CLL cases than other groups regarding TLC and lymphocyte% , this can be explained by increased production and prolonged lifespan of lymphocytes with impaired apoptosis. - There was no significant difference between different study groups in regards to lymphadenopathy, splenomegaly and hepatomegaly of participants .as Non- Hodgkin lymphoma (NHL)is a cancer of the cells of the lymphatic system. NHL can start almost anywhere in the body. It may occur in a single or a group of lymph nodes or an organ as the spleen. lymphoma cells replace the normal cells in the bone marrow, - There was no significant difference between different study groups in regards to CD5 of participants.. p value (0.38), on the other hand, there was a highly significant difference between different study groups in regards to CD23 of participants. P value <0.0001. this explained by CLL cells co-express the T-cell antigen CD5 and B-cell surface antigens CD19, CD20 and CD23. The expression levels of surface immunoglobulin (Ig), CD20 and CD79b are characteristically low compared to those found on normal B cells. - There was highly significant difference between different study groups in regards to CD200 percent, and MFI of CD200 of participants. P value (<0.001). this explained as CD200 expression was first reported in chronic lymphocytic leukemia, where it has a role in differential diagnosis with mantle cell lymphoma . |
