Abstract

Introduction: Myocardial injury after acute UGIB is frequently ignored because the signs and symptoms of ischemia may be overshadowed by those of bleeding.Acute gastrointestinal bleeding can produce hypovolemia, hypotension, and diminished oxygen-carrying capacity causing myocardial ischemia and necrosis. The aim of work: To assess the prevalence and potential risk factors of myocardial injury in patients with liver cirrhosis presenting with acute upper gastrointestinal bleeding. Research Plan: We conducted our study on 132 patients (99 males and 33 females) diagnosed with liver cirrhosis presenting by uppper GIT bleeding at Mansoura University Hospitals during the period from March 2017 to March 2018. Patients were divided into 2 groups according to presence or absence of myocardial injury; group (1) with myocardial injury (76 patients) and group (2) without myocardial injury (60 patients). Results: Troponin I was positive in 25% (33) of patients. ECG ischemic changes were found in 36.3% (48) of patients in the form of ST-segment deviation or T-wave inversion. The incidence of mycoardial injury in our study (elevated troponin levels above cutoff value and/or ECG changes ) was 55% (72) of patients. Conclusion: Predictors of myocardial injury in patients with UGIB included MELD score and variceal source of GI bleeding.
