Abstract

The HSCT is a recent therapeutic method used for hematological malignities. The complications following SCT may affect many organs for example skin , eyes, mouth, intestine, lung and liver, as well as psychological and social wellbeing. Patients’ report a wide range of psychological concerns following SCT that may occur early after discharge from hospital, and continue for many months.
The QoL is an important outcome measured following SCT. Measurements of the QoL can provide information on the need for increased support during and after treatment. Self-care guidelines refers to activities that enable people to deal with the impact of a long term conditions on their daily lives, dealing with the physical and psychosocial changes to give people the opportunity to improve their QoL. Patients with leukemia experience physical, social problems and psychological disorders during and after treatment which can impact their QoL, so those patients are in need for self care guidelines to improve their QoL.

Aim of the study:
This study aimed to evaluate the effect of self-care guidelines on quality of life for patients with leukemia after stem cell transplantation through the following:-
1- Assessing the quality of life dimensions for patients with leukemia after stem cell transplantation pre intervention.
2- Developing and implementing self-care guidelines for patients based on need assessment.
3- Evaluating the effect of the self-care guidelines on quality of life dimensions for patients with leukemia after stem cell transplantation.

Study design:
A quasi experimental design was utilized to meet the aim of the study.
Setting:
The study was conducted at Bone marrow transplantation unit and outpatient clinic at Nasser Institute hospital.
Subjects:
A purposive sample of 60 patients with leukemia after stem cell transplantation was selected according to certain inclusion and exclusion criterias. The study subjects were divided into two groups: study group who had the self-care guidelines & routine care and control group who had only the routine care.
Study tools:
The tools used in this study were:
VI- Patient’s socio-demographic characteristics sheet.
VII- Patient’s clinical data sheet.
VIII- Stem cell transplantation interview questionnaire sheet.
IX- Patients’ self-care practice observational checklists.

Results of this study showed that:
 The majority of the study group (96.7%) had satisfactory level of knowledge, but the minority of control group (3.3%) had satisfactory level of total knowledge about leukemia, SCT procedure and complications post implementation of self-care guidelines with statistically significant difference between them (P<0.001).
 None of patients in the study and control groups had satisfactory level of physical self-care knowledge pre implementation of self-care guidelines while, the majority of patients in the study group (86.7%) had satisfactory level of self-care knowledge regarding physical dimension post implementation of self-care guidelines with highly statistically significant difference between both groups post and follow up of the implementation of self-care guidelines (P<0.001).
 The majority of patients in the study group (90%, 86.7%,86.7% and 86.7%) had satisfactory level of knowledge regarding psychological, social, functional, spiritual and total self-care post implementation of self-care guidelines, but, the minority of control group (6.7%,

6.7%, 6.7%, and 6.7%) had with highly statistically significant difference between them (P<0.001).
 None of patients in the study and control groups had satisfactory level of total knowledge pre implementation of self-care guidelines, while, the majority of study group (90%) had satisfactory level of total knowledge post implementation of self-care guidelines, while none of control group had satisfactory level with statistically significant difference between them. As well, half of study group (50%) while, none of control group had satisfactory level of knowledge follow up of the self-care guidelines implementation with highly statistically significant difference between them (P<0.001).
 None of patients in the study and control groups had satisfactory level of self-care practice pre implementation of self-care guidelines, while, the majority of study group (90%) had satisfactory level of self-care practice post implementation of self-care guidelines, but none of control group had satisfactory level with highly statistically significant difference between them. As well, less than half of study group (43.3%) while, none of control group had satisfactory level of self-care practice at follow up of the self-care guidelines implementation

with highly statistically significant difference between them (P<0.001).
 The results showed that more than two thirds of study group (76.7%) and less than two thirds of control group (63.3%) had low total QoL with no statistically significant differences between two groups’ pre implementation of self-care guidelines (p˃0.05). Also, the majority of study group (96.7%) had high total QoL post self-care guidelines implementation, while, less than two thirds of control group (60.0%) had moderate total QoL. As well, the majority of study group (90%) had high total QoL, while, more than one third of control group (40%) had moderate total QoL. There were statistically significant differences between study and control groups post and follow up implementation of self-care guidelines (P<0.001).
This study concluded that:
Implementation of self-care guidelines has statistically significant positive effect on quality of life dimensions for patients with leukemia after stem cell transplantation.

The current study recommended that:
Recommendation related to the patients:
 Replication of the current study on a larger probability sample is recommended to achieve generalization of the results and wider utilization of the self-care guidelines.
 Self-care guidelines for patients after stem cell transplantation should be applied in all bone marrow transplantation units for all diagnoses and should be up-dated periodically to prevent decline of patients and enhance self-care for those patients.
 A simplified, comprehensive and illustrated booklet including self-care guidelines should be introduced to the patients after stem cell transplantation.
Recommendation related to research:
 Further longitudinal researches are recommended related to patients’ quality of life to evaluate their improvement and prognosis after stem cell transplantation.
