Abstract

Background:
Sepsis is the major cause of morbidity and mortality in pediatric population. Aggressive fluid therapy with catecholamine still the cornerstone in the current treatment protocols in septic shock patients. New drug modalities needed to improve the prognosis.
Objectives:
The aim of the study is to evaluate the role of early intervention with Methylene Blue in management of septic shock patients regarding hemodynamics, duration of septic shock, vasopressor support, morbidity, mortality and length of stay in PICU.
Patients and Method:
Thirty patients fulfilling the criteria of septic shock were randomized into two groups: patients group which received methylene blue bolus at a dose of 1 mg/kg followed after 2 hours by infusion at 0.25mg/kg/h for 4 hours in addition to vasopressors & controls group that received vasopressors plus normal saline.
Results:
The results were favorable in the patients group, which showed higher systolic, diastolic & mean blood pressure and lower pediatric mortality index (PIM) and less vasopressor doses after introduction of methylene blue, but with no effect on mortality.
Conclusion:
It can be concluded that the early use of methylene blue in septic shock patients showed favorable prognosis in comparison with vasopressors only with no effect on mortality
