Abstract

: The commonest primary malignant tumor of the liver is hepatocellular carcinoma (HCC). It represents about 80%-90% of all liver cancers. About 70-90% of HCC cases develop on top of chronic liver disease. Cancer stem cells (CSCs) are defined by self-renewing, differentiation, and tumor-initiating capacities. CSCs are maintained by many regulatory pathways as, Notch. Aim: To evaluate the role of Notch pathway in hepatocarcinogenesis and its effect on hepatic cancer stem cells using immunohistochemical stains, and study their correlations with clinicopathological features and survival. Methods: It was a retrospective study carried out in surgical pathology lab. at gastroenterology Center, Mansoura, Egypt on 100 formalin-fixed paraffin embedded HCC specimens. Tissue microarrays were made by Beecher manual microarrayer. All clinicopathological parameters of these cases were revised. By using tissue microarrays, all cases were immunostained for CD133, EPCAM and Notch1. We assessed the expression of these markers in HCC, and their relations to clinicopathological factors and survival. Results: Many histopathological criteria were found to be significant in differentiation between cases. There was statistically significant correlation between EPCAM and Notch1expression. Conclusion: Notch1 has tumor suppressor role in HCC.
