Abstract

Endoscopic third ventriculostomy(ETV) has become the first-line of treatment for obstructive hydrocephalus,alternative to ventriculoperitonea shunt due to high success rates and low morbidity & mortality rates associated with it.ETV create a direct communication between the third ventricle and the interpeduncular & prepontine subarachnoid spaces, bypassing any
obstruction at the level of the aqueduct of sylvius or fourth ventricle.Assessment of the ETV operation was done by several radiological methods, such as ventricular size changes, using of isotope or contrast ventriculography, and recently examination with cine phase-contrast MRI technique imaging which considered more accurate and reliable method for evaluating CSF dynamics and follow-up of the patient post operative. The aim of this study was to assess the usefulness of a cine phasecontrast MRI for evaluation of patency of ETV stoma and correlation of
these CSF flowmetry data with clinical state post operatively. This study was carried on Twenty patients (11 male, 9 female), whom age range: (4–63 years); mean age 27.4years, with obstructive (non communicating) hydrocephalus who had preoperatively diagnosed by clinical examination & conventional MRI ,then underwent endoscopic third ventriculostomy operation in the neurosurgery department, at Tanta
university hospital between years 2014 and 2017. This study was done in MRI unit of Tanta university diagnostic
radiology & medical imaging department between July 2015& July 2017. In this study cine PC-MRI technique used for qualitative and quantitative assessment of CSF flow dynamics through ETV stoma ,
we demonstrated pulsatile bidirectional motion of CSF flow in mid-sagittal cine image which was highly reliable for ETV patency and for detection of any obstruction by membrane or cyst that indicate impaired function of ETV stoma.
