Abstract

Introduction: Until the 1960s, Intrauterine devices (IUCDs) and condoms were the only artificial method for the control of fertility. The mix of available methods has greatly expanded and the insertion of an IUCD is now the second most prevalent method of family planning used worldwide (13.6%), after female sterilization (20.5%), among women of reproductive age who are married or cohabiting. In Egypt about (33.1%) used IUCD as a method of contraception.
Aim of the work: The aim of this study is to assess the effect of interval and immediate post placental insertion of intrauterine contraceptive devices after cesarean delivery on women’s health.
Patients and Methods: Study design: A randomized controlled clinical trial. Study Site: Ain Shams University Maternity Hospital. Study population: A convenient sample of 300 postpartum women who underwent cesarean delivery have been recruited in the current study, 150 women of them had IUCD inserted immediately post-placental during caesarean delivery, they have been recruited into the immediate post placental group. While the other 150 of them had IUCD inserted after puerperium (6 weeks after cesarean delivery) they have been recruited into post puerperal group.
Results: This study was carried out on 300 postpartum women who underwent cesarean delivery, 150 of them had an IUCD inserted immediately post-placental during caesarean delivery, they were recruited into immediate post placental group. While the other 150 had the IUCD inserted after puerperium (6 weeks after cesarean delivery), they were recruited into the post puerperal group. A total number of 300 cases of women who desired contraception by using an IUCD following cesarean delivery fulfilling recruitment criteria were randomized using computer generated list in a 1:1 ratio into 2 groups: group I women who had immediate post placental IUCD placement. group II women who had an interval IUCD (6 weeks postpartum).
Conclusion: Immediate postplacental caesarean IUCD insertion is highly effective contraceptive methods without significantly increasing the risk of adverse effects such as perforation, bleeding, pain, and infection and convenient for both women and health providers. Expulsion was higher For immediate postplacental IUCD versus interval IUCD. It can contribute significantly to increase the use of IUCD as a long acting reversible contraception in Egyptian population.
Recommendation: Based on the findings of the present study, the following recommendations were suggested: Immediate post placental IUCD insertion is safe and effective family planning contraceptive methods. Further study to evaluate effect of immediate IUCD insertion after normal vaginal delivery. Evaluate patient’s acceptance, satisfaction regarding immediate post placental IUCD insertion. Randomized clinical trials are needed to determine if interval IUCD insertion has any clear benefits over immediate insertion during cesarean delivery.
