Abstract

This study comprised 30 patients suffering from nasal obstruction with accompanying hyponasality due to chronic sinusitis and nasal polyposis. Patients included in the study were not suffering from any ear or throat problems. They did not undergo any previous nasal or sinus surgery before. Patients with major septal deviation were excluded. Patients with uncontrolled hypertension were also excluded as some antihypertensive drugs may give the sensation of nasal stuffiness. Informed consent was taken from all patients to participate in the study. The patients were subjected to full history, clinical examination, especially nasal examination, through anterior rhinoscopy and nasal endoscopy and radiological evaluation by C.T scan to nose and paranasal sinuses to confirm the diagnosis and to assess the severity of the pathology. They were also assessed objectively by using nasometer before the operation and one month after the operation. Functional endoscopic sinus surgery was done under general anethesia. Follow up of the patients was done one month postoperatively. Results were collected, evaluated and subjected to statistical analysis. These patients were 18 males and 12 females. The patients were divided to two groups according to the severity of pathology based on radiological evaluation (group 1 with mild to moderate pathology and group 2 with severe pathology) to assess the effects of such factor on the outcome of surgery. For the same reason, the patients were categorized into three age groups (group1:17-25 ys, group 2 : 26-35 and group 3 : 36-45).
Results of the study showed :
A- Objective improvement of the nasal obstruction after surgery as measured by the mean nasal sentences % by nasometer one month after the operation to match the normal reference values (pre 47.15% and post 58.34%).
B- The changes in the nasal resonance were not affected by the gender or the age of the patient.
C- Improvement in the nasal resonance was more marked in patients with severe pathology than patients with mild to moderate pathology.
Based on the above results, it could be concluded that there is marked improvement in nasality in patients suffering from chronic rhinosinusitis and nasal polyposis treated by functional endoscopic sinus surgery and that such patients, especially professional voice users as teachers and singers, should be informed about such expected improvement.
Finally, it can be safely said that the measurement of nasalance by nasometer is a useful diagnostic procedure in patients with chronic rhinosinusitis with nasal polyposis before and after surgery as it has the advantages of direct assessment, non invasive and non time consuming.
