Abstract

It is well established that DPN is a complex condition that has many
consequences. There are varied clinical presentations of diabetic neuropathy
with involvement of proximal or distal peripheral sensory and motor nerves,
as well as autonomic nerves. The early recognition of neuropathy is
important to reduce morbidity and mortality and to improve quality of life.
This study was conducted on two groups of diabetic patients, either type
1 or type 2, receiving diabetes education and medical treatment selected from
Alagouza charity Hospital between 2015 and 2016.
•group (1): 40 diabetic patients without clinical detectable symptoms
of peripheral neuropathy of short duration (≤ 5 years duration of
diabetes mellitus).
•group (2): 40 diabetic patients with clinical detectable symptoms of
peripheral neuropathy of long duration (>5 years duration of diabetes
mellitus).
Each of the studied groups was subjected to history taking, clinical
examination, laboratory investigations including serum glucose, HbA1c,
serum level of interleukin 6 (IL-6) and neurological tests including nerve
conduction studies, vibration test and monofilament test.
In this study, we compare serum level of IL-6 in diabetic patients with
asymptomatic peripheral neuropathy of short duration (≤ 5 years duration of
diabetes mellitus) and in diabetic patients with symptomatic peripheral
neuropathy of long duration (>5 years duration of diabetes mellitus).
We studied the relation between the relation between nerve conduction
studies of motor and sensory nerves and the serum level of IL-6 in diabetic
patients.
In our study, serum level of IL-6 in diabetic patients with symptomatic
peripheral neuropathy of long duration is higher than diabetic patients with
asymptomatic peripheral neuropathy of short duration.
Serum level of IL-6 shows statistically significant negative correlation
with motor and sensory nerve conduction velocities which is higher in
diabetic patients with symptomatic peripheral neuropathy of long duration
than in diabetic patients with asymptomatic peripheral neuropathy of short
duration.
In our study, it has proven that interleukin 6 (IL-6) has a role for
detection of diabetic peripheral neuropathy.
We suggested that serum level of interleukin 6 (IL-6) can be used as
diagnostic marker for diabetic peripheral neuropathy.
