Cardiothoracic Surgery.All patients in this study were subjected to the following:<br>1- Preoperative and postoperative ECG.<br>2- Preoperative and postoperative echocardiography.<br>3- Laboratory investigations:<br> Routine investigations.<br> Estimation of creatinine.<br> Estimation of serum level of LDH, Troponin and CK-MB.<br> Estimation of serum level of NT-proBNP preoperative<br>and 24 hours after the operation.<br>- NT-Pro BNP was analyzed using a commercial NT-pro BNP<br>ELISA Kit (Biomedica, Vienna, Austria).<br>4- Cardiopulmonary bypass time.<br>5- Aortic cross clamping time.<br>6- Requirement for cardiac support immediately after the operation<br>( drugs or intra-aortic balloon )<br>7- Duration of ventilation dependence<br>8- Duration of ICU and hospital stay.<br>9- Postoperative complications: (neurological- renal-pulmonarybleeding-<br>wound infection)<br>10- Operative mortality: includes in hospital death as well as death<br>within 30 days after surgery.The results of the present study:<br>-We found that there was significant relationship between NTproBNP<br>and need of inotropes after surgery.<br>-Also, our results showed that there was statistically significant<br>difference between pre and postoperative NT-proBNP level.<br>Furthermore, there was significant difference between preoperative<br>and postoperative LDH and CK-MB.<br>-In this study, there was significant relationship between NT-proBNP<br>and duration of ventilation dependence.<br>-We also found significant relationship between NT-proBNP with<br>CBP time and aortic cross clamp time.<br>-As regards outcome, there was significant relationship between NTproBNP<br>with prolonged ICU stay and hospitalization also, there was<br>significant relationship between NT-proBNP and appearance of most<br>postoperative complications. Abstract

Summary and Conclusion
Major cardiac complications after cardiac surgery can
substantially affect outcome (Smith RC et al. 1991).
Early diagnosis and risk stratification are therefore important
issues to develop therapeutic goals that could improve patient care.
Therefore, reliable biologic markers as predictors of the occurrence
of short and long term cardiac complications would be helpful in
routine clinical practice (Jaffe AS et al. 2000).
The B-type natriuretic peptide (BNP) is a neurohormone
mainly secreted from cardiac ventricles by increasing ventricular wall
stress and volume expansion (Hunt PJ et al. 1997).
In response to the appropriate stimulus, BNP is finally cleaved
into BNP and NT-proBNP which is more stable that makes its
measurement more reliable (Hunt PJ et al. 1997).
The objectives of this work were to find out the prognostic
value of NT-proBNP as a valuable marker in predicting postoperative
myocardial insult, short term outcome in patients undergoing open
heart and evaluate the independency of this biomarker.
Thirty five patients who had cardiac surgery in our department
were enrolled in this study their ages ranged from 22 to 68 years, and
they were 14 males and 21 females.
Written informed consent was obtained from all patients
included in the study. The study was approved by the Ethics
Committee of Faculty of Medicine, Tanta University.
