Abstract

Dermoscopy is an inexpensive in vivo and non-invasive technique that permits the visualization of morphologic features of the epidermis and papillary dermis that are not visible to the naked eye. Previously, dermoscopy had been used in differentiating malignant pigmentary disorders, but the use of dermoscopy has recently been extended to the diagnosis of non pigmentary skin disorders by defining thecharacteristic vasculature.Beside to traditional use, dermoscopy is more and more used in the assessment of other generaldermatologic conditions, namely scalp and hair disorders (trichoscopy), nails abnormalities (onychoscopy), skin infections and infestations (entomodermoscopy) and cutaneous inflammatory diseases (inflammoscopy).
Among the list of new applications of dermoscopy, the study of inflammatory dermatoses is probably the most promising topic in terms of development and usefulness, considering the large number of such disorders and the frequent problems in their differential diagnosis which the dermatologist encounters in daily clinical practice.
The main aim of this work was to study the predominant dermoscopic patterns of PP, LP, dermatitis, PR and PLC in an Egyptian sample that included 100 patient with 287 lesions.Dermoscopy used (x10) was connected to a camera with x4 digital zoom rendering the final magnification x40. Dermoscopic evaluation was performed according to:
(i) Background colour (bright red, dull red, light brown, violaceous and yellow)
(ii) Scale colour (white, yellow and both white and yellow )
(iii) Scale distribution (patchy, diffuse, peripheral, central and non scaly).
(iv) Vascular morphology (straight lines, curved lines, straight lines & curved lines , red globules and curved lines & red globules).
(v) Vascular pattern (regular, clustered, patchy, diffuse, peripheral and avascular).
(vi)Pigmentation (absent or present).(vii)=WS (absent or present) and if present we commented on its Morphology (Dotted,radial streaming, reticular,combined (dot and others), circular,linear,veil like and leaf venation pattern).Color (violaceous white, greyish white, yellowish white and homogenous white).
The current study showed significant differences in the dermoscopic patterns of the studied dermatoses which may assist their clinical diagnosis in selected cases.Plaque psoriasis was found to be associated with bright red background color, patchy white scales and regular curved linear and red globules blood vessels. Vascular morphology of curved lines and red globules yielded 80% sensitivity and 92.81% specificity of PP diagnosis. Negative predictors of PP were light brown background color, yellow or peripheral scales, straight linear vascular morphology, absent vascular pattern, present pigmentation and present WS.
