Abstract

Demineralization around fixed appliances during orthodontic treatment period is the most prominent drawback that faces any orthodontics, and sometime lead to the failure of treatment outcomes. Plaque accumulation around orthodontic appliances increases the risk of caries development and the ce readily changed after the brackets omponents of the bacterial plaque arfixation. The aim of this study was to assess the efficiency of an antibacterial monomer-containing adhesive in the reduction of the white spot lesions (WSLs) around orthodontic brackets in-Vivo Compared to the conventional adhesive system.Twenty patient were enrolled who had to extract the maxillary first premolars for orthodontic indications. For each patient, a split mouth design was used, the left maxillary first premolar was served as the control side while the right maxillary first premolar was served as the experimental one. The metal brackets were bonded to the left maxillary first premolars using Transbond XT light cure adhesive (3M Unitek). Meanwhile, they were bonded to the right maxillary first premolars using Clearfil SE protect (kuraray). Instructions were given to the patients regarding brushing two times daily.After three months, both teeth were extracted, and examined by Energy-dispersive X-ray spectroscopy (EDX) and Scanning Electron Microscopy (SEM). The assessment of the calcium & phosphorus atomic percentage was made in the area surrounding the bracket. Mann Whitney test was used for analysis of variance. The results of the study showed a significant difference between the antibacterial monomer-containing adhesive and the conventional adhesive system in the prevention of the WSLs facterial monomer containing adhesormation (P<0.05).Thus the antibive has significant effect in decreasing the enamel demineralization around fixed orthodontic appliances in-Vivo after three months in comparison with the conventional adhesive systems. The antibacterial monomer-containing adhesive (Clearfil SE protect) need more chair time for bonding the orthodontic brackets compared to the conventional system that is less time consuming.
