Abstract

Endometrial carcinoma (EC) is the most common gynecologic malignancy and the seventh neoplasia worldwide. Several techniques have been studied as preoperative tools for EC staging. Magnetic resonance imaging (MRI), computed tomography (CT) and transvaginal sonography (TVS) have all been proven to be accurate in the assessment of depth of myometrial infiltration.
This study aimed to test the accuracy of Ultrasound and 3D power Doppler TVS as preoperative imaging method compared with pelvic MRI.
Our results summarized that:
- This study was carried out on 45 patients with mean age (58.9+5years,). 51.1% of the patient had diabetes and 57.8% had hypertension.44.4% had family history of cancer.
- The majority of the patients were multipara. 20 cases (44.4%) had positive family history toward cancer.
- Regarding tumor-uterine ratio tumor AP / UT AP by U/S in the studied patients, 15 cases (33.3%) show no mass in U/S and revealed Ratio < 50 % was found in 10 cases (22.2%) with myomterial infiltration<50% and Ratio >50 % in 20 cases 44.4% a myometrial infiltration ≥50% (According to Karlsson’s criteria)
- Regarding the adenexa finding by U/S in the studied patients, all cases was free.
- Comparison between different measurements and finding by using MRI, U/S and pathology as regarding uterine size ,tumor size endometrial thickness, myomterial and cervical invasion ,nodal involvement, adenexa and Ascites.
- Regarding the sensitivity, specificity of both U/S and MRI in detect the cervical extension in relation to pathological findings, the sensitivity of U/S in detect cervical extension was 28.6% and in MRI was 95.7, the specificity of U/S was 97.3 and the MRI was 92.1%, the positive predictive value of U/S was 66.7% while the MRI was 66.7%, the negative predictive value of U/S was 88.0% and MRI was 97.2%, finally the accuracy of U/S was 86.7% and MRI was 91.1%.
- Regarding the sensitivity, specificity of both U/S and MRI in detect the tumor size in relation to pathological findings, the sensitivity of U/S in detect tumor size was 83.3% and in MRI was 91.7, the specificity of U/S was 84.8 and the MRI was 90.9%, the positive predictive value of U/S was 66.7% while the MRI was 78.6%, the negative predictive value of U/S was 93.3% and MRI was 96.7%, finally the accuracy of U/S was 89.4% and MRI was 91.1%.
- Regarding the sensitivity, specificity of both U/S and MRI in detect the lymph node in relation to pathological findings, the sensitivity of U/S in detect lymph node was 40.0%and in MRI was 66.7%, the specificity of U/S was 86.7 and the MRI was 93.3%, the positive predictive value of U/S was 85.7% while the MRI was 83.3%, the negative predictive value of U/S was 76.3% and MRI was 84.8%, finally the accuracy of U/S was 77.8% and MRI was 84.4%.
- Regarding the sensitivity, specificity of both U/S and MRI in detect the myometrium invasion in relation to pathological findings, the sensitivity of U/S in detect myometrium invasion was 85.7% and in MRI was 88.1 the specificity of U/S was 81.7 and the MRI was 82.0%, the positive predictive value of U/S was 94.0% while the MRI was 95.0%, the negative predictive value of U/S was 88.0% and MRI was 86.0%, finally the accuracy of U/S was 86.0% and MRI was 87.1%.
