Abstract

This study was conducted on 71 patients with classical trigeminal neuralgia.They were divided into 3 groups. group 1 included 23 patients and they underwent microvascular decompression(MVD)of the 5th nerve at it REZ.group 2 included 26 patients who were operated using percutaneous balloon compression(PBC)of the trigeminal ganglion. group 3 included 22 patients and they were managed using percutaneous radiofrequency ablation (RFA) of the trigeminal ganglion.Patients selected for our study were those with classical TN who have recurrent attacks of typical lancinating pain along distribution of one or more of trigeminal divisions and showed no satisfaction with medical treatment.MRI scans were done for all patients to exclude causes of secondary TN. The pain intensity was assessed for all patients using the visual analogue scale (VAS). 3 scores were obtained for each patient, first just before operation,second immediate after recovery,and lastly at the end of the follow up period. For group 1 (MVD), the mean age was 61.09 years (range 50-70).Female patients represented 56.6% of patients. The right side was affected in 65.2% of cases. Combined V2 and V3 affection represented 34.8% of patients,followed by V3(21.7%)and lastly by combined V1 and V2(17.4%).The mean duration of preoperative complaint was 8.17 years (range 1-20). We achieved an initial pain relief (either partial or complete) in 100% of cases with a mean VAS of 0.3. MVD had achieved 96.52% reduction in pain intensity immediately postoperative.This reduction in pain intensity reached 92.93% at the end of follow up period. The average rate of the pain free/follow up period was 96%.Reported complications included transient CSF leak in 2 cases and transient facial nerve palsy in 1 case.
