Abstract

As the elderly population grows, the number of hip fractures continues to increase. The elderly have weaker bone and are more likely to fall due to poorer balance, medication side effects, and difficulty maneuvering around environmental hazards. Nurses are involved in caring for patients with hip fractures and should be familiar with the basic assessment and management of these injuries.
Aim of the study:
This research was designed to identify the impact of rehabilitation program for elderly patients with hip fracture at Sohag University Hospital.
Research design:
Quasi-experimental research design was used in this study.
Setting:
The study was conducted inpatient orthopedic department and follow up in the orthopedic outpatient clinic at Sohag University Hospital.
Subjects:-
The study included a convenience sample of elderly patients attending to orthopedic department for one year who had hip fracture, their number were (76) elderly person aged 60 years and above. They were divided into two groups study group and control group the equal number for each one (38).
The following tools were utilized for data collection:
Tool I: structured interview questionnaire: It was developed by the researcher to collect the necessary data. It consisted of four parts:
Part (I): Personal patient characteristics.
Part (II): Medical history; full history was taken from elderly which consisted of assessment of past & present history.
Part (III): Assessment of walking condition before fracture & after the fracture.
Part (IV): Assessment of pain level using Wong-Baker Face Rating Scale.
Tool II: Assessment of patient’s knowledge & practices, it includes the following two parts:
Part (I): Assessment of patient’s knowledge regarding hip fracture

Part (II): Assessment of patient’s practices regarding how to deal with hip fracture and problems.
Tool III: The Modified Barthel Index (MBI) is designed to determine the degree of independence in activities daily living (ADLs).
Main findings of the present study were as the follows:
 Nearly two thirds of the patients had ranged between age 60 to 70 years, slight more than half of them were females, the majority of them uneducated, also worked without cash, more than half of them married and the majority of were living with family.
 Cardiovascular diseases are most prevalent diseases among the studied sample; in addition more than one fifth of them had previous fracture.
 The most common type of hip fracture among studied sample is intertrochanteric fracture; main cause of hip fracture is fall down, high percentage of studied able to walk without any assistant before hip fracture.
 There is significant differences in improvements throughout rehabilitation program among study group regarding total score of knowledge & practice, reduce incidence of complications among study group in addition improve walking ability and level of dependency, while the majority of the studied sample not return pre-fracture condition.
The following recommendations are suggested:
1- Increase public awareness about hip fracture through the mass media. Also the need for expanded health education about the problems for elders in rural areas of Sohag that can be achieved by health classes and home visits.
2- Periodic checkup of bone mineral density on physical examination for the elderly people is a requirement for the prevention hip fracture.
3- Booklet for rehabilitation instruction is great importance for the patients and their relative.
4- A specialized nurse rehabilitator is to be full time attending the orthopedic department and outpatient clinic to instruct the patients of the rehabilitation instructions needed
